ABSTRACT

La cavidad oral se puede ver alterada a causa del virus SARS-CoV-2 de manera transitoria o permanente, lo que afecta a distintas partes de la cavidad oral incluyendo las glándulas salivales, esto causa xerostomía. La disgeusia es uno de los síntomas principales a lo largo de la enfermedad, nos enfocaremos en aquellas manifestaciones psicológicas como la ansiedad y el estrés en su interacción con la aparición del trastorno llamado bruxismo (AU)


The oral cavity can be altered by the SARS-CoV-2 virus in a transient or permanent manner, affecting different parts of the oral cavity including salivary glands causing xerostomia. Dysgeusia is one of the main symptoms throughout the disease, we will focus on those psychological manifestations such as anxiety and stress in their interaction with the appearance of the disorder called bruxism (AU)

de la deglución, los cuales representan todas las alteraciones del proceso fisiológico encargado de llevar el alimento desde la boca al esófago y después al estómago, salvaguardando siempre la protección de las vías respiratorias. OBJETIVO. Definir el manejo óptimo, de la disfagia en pacientes con antecedente de infección severa por COVID-19. METODOLOGÍA. Se realizó una revisión de la literatura científica en las bases de datos PubMed y Elsevier que relacionan el manejo de la disfagia y pacientes con antecedente de infección severa por SARS-CoV-2. Se obtuvo un universo de 134 artículos que cumplieron los criterios de búsqueda. Se seleccionaron 24 documentos, para ser considerados en este estudio. RESULTADOS. La incidencia de disfagia posterior a infección severa por SARS-CoV-2 fue del 23,14%, siendo la disfagia leve la más frecuente 48,0%. Los tratamientos clínicos más empleados en el manejo de la disfagia fueron rehabilitación oral y cambio de textura en la dieta en el 77,23% de los casos, mientras que el único tratamiento quirúrgico empleado fue la traqueotomía 37,31%. Un 12,68% de pacientes recuperó su función deglutoria sin un tratamiento específico. La eficacia de los tratamientos clínicos y quirúrgicos en los pacientes sobrevivientes de la infección severa por SARS-CoV-2 fue del 80,68%, con una media en el tiempo de resolución de 58 días. CONCLUSIÓN. La anamnesis es clave para el diagnóstico de disfagia post COVID-19. El tratamiento puede variar, desde un manejo conservador como cambios en la textura de la dieta hasta tratamientos más invasivos como traqueotomía para mejorar la función deglutoria.


INTRODUCTION. The difficulty to swallow or dysphagia is included within the problems of swallowing, which represent all the alterations of the physiological process in charge of carrying the food from the mouth to the esophagus, and then to the stomach, always taking into account the protection of the airways. OBJECTIVE. To define the optimal management, both clinical and surgical, for the adequate treatment of dysphagia produced as a consequence of severe SARS-CoV-2 infection. METHODOLOGY. A review of the scientific literature was carried out using both PubMed and Elsevier databases, which relate the management of dysphagia and patients with a history of severe SARS-CoV-2 infection. RESULTS. The incidence of dysphagia following severe SARS-CoV-2 infection was of 23,14%, with mild dysphagia being the most frequent 48,00%. The most frequently used clinical treatments for dysphagia management were oral rehabilitation and change in dietary texture in 77,23% of cases, while tracheotomy was the only surgical treatment used 37,31%. A total of 12,68% of patients recovered their swallowing function without specific treatment. The efficacy of clinical and surgical treatments in survivors of severe SARS-CoV-2 infection was 80,68%, with a mean resolution time of 58 days. CONCLUSION. An adequate medical history is key to the diagnosis of post-COVID-19 dysphagia. Treatment can range from conservative management such as changes in diet texture to more invasive treatments such as tracheotomy to improve swallowing function.

Xerostomia is defined as the perception of dry mouth, and dysgeusia, as a change in taste. Both are common complaints in the elderly, especially among those making use of polypharmacy drug combinations. Aim: This study aimed to determine the prevalence of xerostomia and dysgeusia and to investigate their association with polypharmacy in the elderly. Methods: older people under follow-up at the Multidisciplinary Elderly Center of the University Hospital of Brasília were interviewed and asked about health problems, medications used, presence of xerostomia and dysgeusia. Descriptive statistics were used to determine the prevalence of the symptoms surveyed. The chi-square test was used to investigate the relationship between xerostomia and dysgeusia and polypharmacy. Secondary associations were performed using binomial logistic regression. Results: Ninety-six older people were evaluated and of these, 62.5% had xerostomia and 21.1%, had dysgeusia. The average number of medications used was 4±3 medications per individual. Polypharmacy was associated with xerostomia but not dysgeusia. It was possible to associate xerostomia with the use of antihypertensive drugs. Conclusion: Xerostomia was a frequent complaint among elderly people making use of polypharmacy, especially those using antihypertensives. Antihypertensives and antidepressants were used most drugs by the elderly and exhibited interactions with drugs most prescribed in Dentistry. Two contraindications were found between fluconazole and mirtazapine; and between erythromycin and simvastatin

ABSTRACT

As alterações do paladar durante o transplante de células tronco hematopoiéticas (TCTH) são frequentemente descritas em períodos após a finalização do transplante, porém pouco se conhece sobre quais sabores e que tipo de alterações de percepção são vivenciadas pelos pacientes durante o transplante. O objetivo deste trabalho foi caracterizar as alterações do paladar e de despapilação presentes no período de neutropenia do TCTH, bem como verificar se há associação entre essas alterações e variáveis clínicas relacionadas ao transplante e à toxicidade na mucosa oral. Também se avaliou o impacto das alterações do paladar e da mucosa oral na qualidade de vida dos pacientes. Foram selecionados 52 pacientes de TCTH autólogo e alogênico, que realizaram testes de acuidade do paladar antes do condicionamento e durante a neutropenia. Os pacientes foram ainda submetidos a oroscopia para avaliação de mucosite oral e despapilação no dorso da língua. Foi também realizado inquérito quanto a xerostomia e a alterações no paladar. Foi realizado teste de acuidade na percepção dos sabores doce, amargo, azedo e salgado, ofertados em soluções contendo concentrações baixas e altas de substâncias estimuladoras desses sabores. A qualidade de vida foi avaliada pelo questionário EORTC QLQ-C30 e QLQ-H&N35. Detectou-se que os 46,1% dos pacientes exibiram alterações do paladar antes do condicionamento do TCTH, mas que essa frequência aumentou para 90,5% durante a neutropenia (teste do c2, p=0.042). O tipo de alteração mais comum foi a hipogeusia dos sabores de concentração mais forte, principalmente do amargo. Alta frequência de pacientes (72,0%) foi detectada exibindo despapilação no período de neutropenia, porém essa despapilação não foi associada às alterações de percepção de cada tipo de sabor. Não houve associação entre alterações do paladar e tipo de transplante, tipo de condicionamento e variáveis relacionadas à toxicidade na cavidade oral e no trato gastrointestinal. Houve associação significativa entre despapilação e duração da mucosite oral >=8 dias (OR= 5,62, IC95% = 0,98-60,30, p=0.039). As alterações salivares e do paladar reduziram significativamente na qualidade de vida durante a neutropenia em comparação ao período que antecedeu o condicionamento. Concluiu-se que as alterações do paladar já estão presentes antes do TCTH, mas há aumento da frequência dessas alterações, principalmente de hipogeusia. A despapilação na língua ocorreu após o condicionamento, e foi associada a maior tempo de duração da mucosite oral. O impacto das alterações salivares e do paladar na qualidade de vida do paciente durante a neutropenia é alto e devem ser minimizados mediante a adoção de estratégias mais abrangentes, que incluam manutenção da integridade da mucosa oral.

ABSTRACT

Resumen Los trastornos del gusto constituyen un grupo de alteraciones relativamente frecuentes en la práctica clínica. Son de etiología diversa, pudiendo afectar la vía gustativa en distintos niveles, manifestándose, clínicamente, con alteraciones cuantitativas y/o cualitativas de la sensibilidad gustativa. En gran parte de los casos se puede lograr un diagnóstico etiológico con una anamnesis adecuada, examen físico y exploraciones complementarias básicas. No obstante, existe poco entendimiento de su patogénesis, así como limitadas opciones terapéuticas, lo que implica la falta de algoritmos de diagnóstico y tratamiento completamente validados. El objetivo de la presente revisión es otorgar una actualización y un abordaje práctico de los trastornos del gusto.


Abstract Taste disorders represent a group of alterations relatively frequent in clinical practice. They have a diverse etiology, and they can affect the gustatory pathway at different levels, clinically manifesting with quantitative/qualitative alterations on taste sensitivity. In most of cases it is possibly to achieve an etiologically diagnosis with appropriate anamnesis, physical exploration, and basic complementary tests. However, their pathogenesis is not well understood, and they have limited therapeutic options, which determines a lack of well-validated diagnosis and treatment algorithms. The objective of this review is to provide and update and a practical approach to taste disorders.

ABSTRACT

Introdução: O manejo de pacientes com ardor bucal é um desafio no cotidiano clínico da odontologia. Objetivo: Comparar o efeito da Terapia a Laser de Baixa Intensidade (LLLT) e da Estimulação Elétrica Nervosa Transcutânea (TENS) no tratamento do ardor bucal. Metodologia: Ensaio clínico randomizado constituído por 25 pacientes com ardor bucal que foram tratados por TENS (n=12) e por LLLT (n=13). Os protocolos de tratamento foram aplicados semanalmente por 8 semanas. O teste análise de variância (ANOVA) dois fatores foi usado para verificar se existia diferença significativa entre os tempos T0 (antes de iniciar o tratamento), T1 (após a 4ª sessão de tratamento), T2 (após a 8ª sessão de tratamento) e T3 (30 dias após o término do tratamento) em relação aos sintomas, analisados por meio da Escala Visual Analógica (EVA), fluxo salivar não estimulado, xerostomia e disgeusia com as intervenções de TENS e LLLT. Resultados: A maioria dos pacientes foi do sexo feminino no período pós-menopausa com média de idade no grupo TENS de 59,25 anos e no grupo LLLT de 62,08. Hipertensão e dislipidemia foram as alterações sistêmicas mais frequentes. Ansiedade e depressão foram os únicos transtornos psiquiátricos relados. A maioria dos pacientes fazia uso de medicamentos como anti-hipertensivos e antidepressivos. Não foram observadas variações expressivas no que se refere a xerostomia e a disgeusia nos dois grupos analisados. A TENS e a LLLT foram eficazes na redução dos sintomas relatados pelos pacientes (p˂0,001), entretanto, observou-se entre os tempos T2 e T3 que o grupo LLLT apresentou uma melhor resposta quando comparado ao TENS (p=0,003). Os pacientes do grupo TENS apresentaram aumento do fluxo salivar entre os tempos T1 e T2, enquanto o grupo LLLT apresentou uma diminuição (p=0,052). Conclusão: A TENS e a LLLT foram eficazes na redução dos sintomas do ardor bucal durante o tratamento e 30 dias após o término do tratamento, sendo que o grupo LLLT apresentou uma melhor resposta na sessão de acompanhamento pós-tratamento quando comparado ao grupo TENS (AU).


Introduction: The management of patients with burning mouth is a challenge in the clinical routine of dentistry. Objective: To compare the effect of Low Intensity Laser Therapy (LLLT) and Transcutaneous Electrical Nerve Stimulation (TENS) in the treatment of burning mouth. Methodology: Randomized clinical trial consisting of 25 patients with burning mouth who were treated with TENS (n=12) and LLLT (n=13). Treatment protocols were applied weekly for 8 weeks. The two-way analysis of variance (ANOVA) test was used to verify whether there was a significant difference between the times T0 (before starting treatment), T1 (after the 4th treatment session), T2 (after the 8th treatment session) and T3 (30 days after the end of treatment) in relation to symptoms, analyzed using the Visual Analogue Scale (VAS), unstimulated salivary flow, xerostomia and dysgeusia with TENS and LLLT interventions. Results: Most patients were female in the postmenopausal period, with a mean age of 59.25 years in the TENS group and 62.08 in the LLLT group. Hypertension and dyslipidemia were the most frequent systemic alterations. Anxiety and depression were the only psychiatric disorders reported. Most patients used drugs such as antihypertensives and antidepressants. Significant variations were not observed with regard to xerostomia and dysgeusia in the two groups analyzed. TENS and LLLT were effective in reducing the symptoms reported by patients (p˂0.001), however, it was observed between times T2 and T3 that the LLLT group showed a better response when compared to TENS (p=0.003). Patients in the TENS group showed an increase in salivary flow between times T1 and T2, while the LLLT group showed a decrease (p=0.052). Conclusion: TENS and LLLT were effective in reducing the symptoms of burning mouth during treatment and 30 days after the end of treatment, and the LLLT group showed a better response in the posttreatment follow-up session when compared to the TENS group (AU).

ABSTRACT

Objetivo: Determinar la prevalencia de trastornos del gusto en pacientes niños y adolescentes diagnosticados con infección por coronavirus, a través de la evidencia en la literatura científica. Materiales y métodos: Una revisión sistemática de los artículos publicados entre el 19 de diciembre del 2019 y el 20 de diciembre del 2020 en las bases de datos Medline, Lilacs, BVS, Cochrane, SCOPUS y ScienceDirect. La estrategia de búsqueda de información se basó en el diagrama de flujo clásico de PRISMA. Para la evaluación del riesgo de sesgo, se usó la escala Newcastle-Ottawa. Resultados: Se encontraron 443 artículos en seis bases de datos y se incluyó un total de 7 artículos después de la evaluación, según los criterios de selección. Los artículos abordaron la variable de trastornos del gusto en tres formas: ageusia, disgeusia e hipogeusia, y se halló que esta manifestación clínica estaba presente desde los inicios de la infección. Conclusiones: Se ha encontrado una prevalencia de trastornos del gusto en niños y adolescentes diagnosticados con infección por coronavirus desde un 3,3% hasta un 26,9%. (AU)


Aim: To assess the prevalence of taste disorders in children and adolescents diagnosed with coronavirus infection according to the evidence reported in the scientific literature. Materials and Methods: A systematic review of articles published between December 19, 2019, and December 20, 2020 in the Medline, Lilacs, BVS, Cochrane, SCOPUS and ScienceDirect databases. The information search strategy was based on the classic PRISMA flow diagram. The Newcastle-Ottawa scale was used to assess the risk of bias. Results: 443 articles were found in six databases, and a total of 7 articles were included after evaluation according to the selection criteria. The articles addressed the variable of taste disorders in three ways: ageusia, dysgeusia and hypogeusia; finding that this clinical manifestation was present from the beginning of the infection. Conclusions: The prevalence of taste disorders in children and adolescents diagnosed with coronavirus infection is from 3.3% to 26.9%. (AU)

RESUMEN Introducción: El estudio ecocardiográfico mediante speckle-tracking ha emergido como un novedoso método para la evaluación cuantitativa de la función miocárdica y se ha correlacionado con la presencia de enfermedad coronaria. Objetivo: Determinar el valor pronóstico de la deformación miocárdica (strain) por speckle-tracking bidimensional en la evolución de pacientes con sospecha de cardiopatía isquémica. Método: Estudio longitudinal prospectivo analítico con 51 pacientes a los que se realizó ecocardiograma con evaluación de la deformación (strain) longitudinal global por speckle-tracking bidimensional y coronariografía en el CIMEQ entre 2016 y 2018. Se siguieron los eventos cardíacos graves (infarto de miocardio, muerte cardíaca y por otras causas, enfermedad cerebrovascular y necesidad de nueva revascularización). Resultados: El 70,6% de los enfermos resultaron ser del sexo masculino, el 81,4% mayores de 50 años y 81,4%, fumadores. El 65,0% tenía enfermedad coronaria significativa y 55,0% de los casos fueron revascularizados. Se registraron 4 muertes (7,8%) y un infarto no fatal (2,0%). Los pacientes con deformación longitudinal global ≤ -15% (en valores absolutos) tuvieron mayor frecuencia de eventos cardíacos graves (p=0,02). Conclusiones: La deformación longitudinal global medida por speckle-tracking bidimensional no pudo predecir la presencia de enfermedad coronaria, pero sí los eventos cardiovasculares graves en los enfermos con enfermedad coronaria significativa.


ABSTRACT Introduction: Two-dimensional speckle-tracking echocardiography has emerged as a novel method for the quantitative evaluation of myocardial function and it has been correlated with the presence of coronary artery disease. Objective: To determine the prognostic value of myocardial strain by two-dimensional speckle-tracking echocardiography in the evolution of patients with suspected ischemic heart disease. Method: An analytical prospective longitudinal study was carried out with 51 patients, who underwent global longitudinal strain assessment by two-dimensional speckle-tracking echocardiography, and coronary angiography at CIMEQ between 2016 and 2018. Major cardiac adverse events (myocardial infarction, cardiac and non-cardiac cause of death, cerebrovascular disease, and need for new revascularization) were followed. Results: The 70.6% of patients were male, the 81.4% were older than 50 years old, and the 81.4% were smokers. The 65.0% of patients had significant coronary artery disease and the 55.0% underwent myocardial revascularization. There were four deaths (7.8%) and one non-fatal myocardial infarction (2.0%). Patients with global longitudinal strain ≤ -15% (in absolute or modular values) had a higher frequency of major adverse cardiovascular events (p=0.02). Conclusions: Global longitudinal strain by two-dimensional speckle-tracking echocardiography could not predict the presence of coronary artery disease, but it did predict major adverse cardiovascular events in patients with significant coronary artery disease.

ABSTRACT

@#The authors report a case of post-tonsillectomy dysgeusia and discuss the pathogenesis, diagnostics, as well as treatment options done in several reported cases. A 37-year-old man who was diagnosed with recurrent tonsillitis underwent bilateral palatine tonsillectomy, and on the second post-operative day, post-tonsillectomy hemorrhage ensued which required emergency hemostasis at the operating room. Intra-operative findings include active bleeding on the left tongue base, wherein hemostasis was achieved via electrodissection. After the procedure, patient noted a disturbance to taste that persisted for several months. Dysgeusia is an unusual complication of tonsillectomy, occurring in 0.3% to 9% of cases.

Introdução: O tratamento antineoplásico pode causar efeitos colaterais que prejudicam a ingestão alimentar e o estado nutricional dos pacientes. Objetivo: Avaliar a percepção do paladar de pacientes em tratamento oncológico, relacioná-la com as variáveis pessoais e clínicas e compará-la com a percepção do paladar de indivíduos sadios. Método: Trata-se de um estudo transversal com 50 indivíduos em tratamento oncológico e outros 50 indivíduos do grupo controle. Para avaliar a percepção do paladar, os participantes receberam amostras de alimentos e foram questionados quanto aos sabores doce, amargo, salgado e azedo. A intensidade do sabor foi avaliada por meio de uma escala de 1 a 5. Resultados: Os pacientes em tratamento oncológico perceberam os sabores dos seguintes alimentos de maneira mais acentuada: doce de leite (4,48±1,0 vs. 3,78±0,9; p<0,05), goiabada (4,10±1,0 vs. 3,52±0,9; p<0,05), suco de limão (4,42±0,9 vs. 2,86±1,0; p<0,05), suco de maracujá (3,76±1,2 vs. 2,56±1,0; p<0,05) e café (4,42±0,8 vs. 3,14±0,9; p<0,05). Já os alimentos: biscoito de polvilho, iogurte natural, rúcula e chicória foram percebidos com menor intensidade quando comparados ao grupo controle. Entre as quatro categorias de sabor avaliadas, os alimentos do grupo salgado foram os que menos se diferenciaram quanto à percepção do paladar e os alimentos azedos e amargos foram percebidos de forma mais distinta entre os grupos. Conclusão: O presente estudo é pioneiro em avaliar a percepção do paladar utilizando alimentos, ao invés de soluções padronizadas; e, por meio dessa técnica, identificaram-se diferenças significativas na intensidade da percepção do paladar dos indivíduos em tratamento oncológico.


Introduction: Antineoplastic treatment can cause side effects that impair the patients' food intake and nutritional status. Objective: To evaluate the taste perception in patients undergoing cancer treatment, to relate it to personal and clinical variables and to compare with taste perception in healthy individuals. Method: Cross-sectional study with 50 individuals undergoing cancer treatment and another 50 individuals in the control group. To assess taste perception, participants received food samples and were asked about sweet, bitter, salty and sour flavors. The intensity of the flavor was assessed using a scale from 1 to 5. Results: Patients undergoing cancer treatment perceived the flavors of the following foods more strongly: dulce de leche (4.48±1.0 vs. 3.78±0.9; p<0.05), guava (4.10±1.0 vs. 3.52±0.9; p<0.05), lemon juice (4.42±0.9 vs. 2.86±1.0; p<0.05), passion fruit juice (3.76±1.2 vs. 2.56±1.0; p<0.05) and coffee (4.42±0.8 vs. 3.14±0.9; p<0.05). The foods: cassava starch, natural yogurt, arugula and chicory were perceived with less intensity when compared to the control group. Among the four flavor categories evaluated, the foods in the salty group were the ones that least differed in terms of the taste perception and sour and bitter foods were perceived more distinctly between the groups. Conclusion: The present study is pioneer in evaluating the taste perception using food, instead of standardized solutions, and through this technique, significant differences were identified in the intensity of taste perception of individuals undergoing cancer treatment.


Introducción: El tratamiento antineoplásico puede causar efectos secundarios que perjudican la ingesta de alimentos y el estado nutricional de los pacientes. Objetivo: Evaluar la percepción del gusto en pacientes sometidos a tratamiento contra el cáncer, relacionarlo con variables personales y clínicas y compararlo con la percepción del gusto en individuos sanos. Método: Este es un estudio transversal con 50 individuos en tratamiento contra el cáncer y otros 50 individuos en el grupo de control. Para evaluar la percepción del sabor, los participantes recibieron las muestras de alimentos y se les preguntó sobre los sabores dulces, amargos, salados y agrios. La intensidad del sabor se evaluó usando una escala del 1 al 5. Resultados: Los pacientes en tratamiento contra el cáncer percibieron los sabores de los siguientes alimentos con más fuerza: dulce de leche (4,48±1,0 vs. 3,78±0,9; p<0,05), guayaba (4,10±1,0 vs. 3,52±0,9; p<0,05), jugo de limón (4,42±0,9 vs. 2,86±1,0; p<0,05), jugo de maracuyá (3,76±1,2 vs. 2,56±1,0; p<0,05) y café (4,42±0,8 vs. 3,14±0,9; p<0,05). Los alimentos: galletas de almidón, yogur, rúcula y achicoria se percibieron con menos intensidad en comparación con el grupo de control. Entre los sabores, los alimentos salados fueron los que menos diferían y los alimentos amargos y amargos se percibieron más claramente entre los grupos. Conclusión: El presente estudio es pionero en evaluar la percepción del gusto utilizando alimentos, en lugar de soluciones estandarizadas, y identificó diferencias significativas en la intensidad de la percepción del gusto en individuos en tratamiento contra el cáncer.

Em uma linguagem acessível e usando ótimas ilustrações e tabelas, esclarece sobre os cuidados nutricionais e também sobre alguns alimentos e nutrientes importantes para a imunidade. In an accessible language and using great illustrations and tables, it clarifies about nutritional care and also about some foods and nutrients important for immunity. Instructs you how to do it in situations of poor appetite, loss of taste (dysgeusia), difficulty swallowing (dysphagia), loss of smell (anosmia) and diarrhea. It advises on food purchases, on the risks of contamination, expiration time and storage in the refrigerator and freezer, as well as care in preparing them. It presents instructions on hand hygiene with soap and water and how to rub hands with alcoholic preparations, on proper handling of waste removal and instructions for diluting the chlorinated solution to surfaces and to packaging and food. INTRODUCCIÓN: El personal de salud, entre ellos los médicos, es parte fundamental en primera línea de defensa ante la pandemia de COVID-19, causada por SARS-Cov-2. OBJETIVO: Caracterizar la clínica y evolución de los primeros casos de contagio por coronavirus en médicos de Perú. METODOLOGÍA: Se presentan una serie de seis casos de médicos infectados por coronavirus, con positividad confirmada para COVID-19, mostrando la evolución diaria desde el diagnóstico de la enfermedad, sus principales signos y síntomas, la evolución de los mismos y hasta el desenlace en cada caso. RESULTADOS: De los casos estudiados, cinco fueron hombres, tenían una mediana de edad de 28 años (rango intercuartílico: 27 a 33). Tres de ellos trabajaban más de 12 horas al día en servicios de hospitalización y emergencia; y tres no contaban con mascarilla como método de protección personal. Los síntomas más frecuentes fueron la temperatura axilar superior a 38 grados Celsius, el malestar general, la tos seca y la odinofagia (este último en tres pacientes). En cuanto al diagnóstico con la prueba molecular, tuvo una mediana de tres días de demora (con rango: de 2 a 6 días). Los síntomas que más persistieron fueron la tos seca (presente durante 10 días en cuatro médicos), y la disgeusia como síntoma único, que tuvo la mayor duración (15 días en un solo médico). En los seis casos la evolución fue favorable. Sin embargo, aún se tienen deficiencias para la definición de reincorporación laboral a sus centros hospitalarios. DISCUSIÓN: A pesar de ser un número pequeño de casos, es el primer reporte en personal de salud y que detalla día a día la evolución de los síntomas de COVID-19. Esto puede servir para la salud ocupacional, e incluso como base para la vigilancia y monitorización de los casos en una población mayor.


INTRODUCTION: Health personnel, including physicians, are a fundamental part of the first line of defense against the SARS-CoV-2 pandemic. OBJECTIVE: To characterize the clinical manifestations and course of the first cases of contagion by SARS-CoV-2 in doctors of Peru. METHODOLOGY: We present a series of six cases of doctors infected by SARS-CoV-2, with confirmed positivity for COVID-19, showing the daily evolution from the diagnosis of the disease, its main signs and symptoms, evolution, and until the outcome in each case. RESULTS: Five were men. The median age was 28 years (interquartile range: 27 to 33). In three cases the physician worked more than 12 hours a day in emergency and hospitalization services and not wear a mask at all times. The most frequent symptoms were axillary temperature above 38°C, malaise, dry cough, and odynophagia (the latter in three of the cases). The diagnosis was made at a median of 3 days (interquartile range: 3 to 4 days). The symptoms that persisted the most were dry cough (present during ten days in four doctors). Dysgeusia was the only symptom with the most extended duration (15 days in only one doctor). In the six cases, the course was favorable. However, these doctors found it difficult to return to functions in their hospital centers adequately. DISCUSSION: Despite a small number of cases, it is the first report detailing the evolution of symptoms day by day, which can help for occupational health and even for case surveillance and monitoring.

BACKGROUND: Bitterness perception seems to be related to an enhanced intake of dietary fat and to a tendency to the development of diseases such as obesity. However, the exact factors for this possible contribution still need to be better investigated. So, gustatory perception of the bitter taste is a promising area of study because of its importance regarding food choices and consequently feeding behavior. Therefore, this short review focused on recent papers reporting correlations between bitter taste, anthropometric variables, obesity and other chronic diseases, age, gender, ethnicity, and genetics. METHODS: A survey was performed in MEDLINE (PubMed) and Scielo from September 2015 to January 2017.Only review articles, observational studies and clinical trials published in English and Portuguese over the last15 years which met the objectives of the present study were considered. A total of 40 papers were evaluated. RESULTS: Two papers showed a positive correlation between bitter taste and obesity, one indicated that this correlation is influenced by the subject's age, one suggested a negative correlation, and two found no association. Age seems to be negatively correlated with the bitterness perceived, and female gender was associated with a stronger perception of bitterness. Genetics, mostly due to differences in TAS2R38 expression, influences sensitivity to the bitter taste, feeding behavior and also alcohol intake. Ethnicity, not only the subject's phenotypic or genotypic characteristics, seems to play a role in taste perception and nutritional diseases. CONCLUSIONS: Age, gender, genetics and ethnicity seem to play a role in bitterness perception. Data about associations between bitterness perception and anthropometrics are conflicting.

ABSTRACT

OBJECTIVE: To determine the therapeutic effects of two selective GABA-A agonists, zopiclone and eszopiclone, in the treatment of insomnia. METHODS: This study comprised a phase III, single-center, randomized, double-blind, double-dummy, parallel-group, non-inferiority trial. Patients were randomized to receive zopiclone 7.5 mg or eszopiclone 3 mg, both orally, for four weeks. In total, 199 patients were evaluated during two visits and then followed for at least six weeks. The primary endpoint was the Insomnia Severity Index after four weeks of treatment. Secondary endpoints were obtained through polysomnography data, including total sleep time, sleep latency and sleep efficiency. The frequency of adverse events was also analyzed. ClinicalTrials.gov: NCT01100164. RESULTS: The primary efficacy analysis demonstrated the non-inferiority of eszopiclone over zopiclone. Analysis of objective parameters assessed by polysomnography showed that eszopiclone increased total sleep time and also improved sleep efficiency. The safety profile of both study treatments was similar and the most common events reported in both groups were dysgeusia, headache, dizziness, irritability and nausea. Adverse events were observed in 223 patients, 109 (85.2%) in the eszopiclone group and 114 (87.7%) in the zopiclone group. CONCLUSION: Based on the Insomnia Severity Index at the end of four weeks of treatment, eszopiclone demonstrated efficacy comparable to that of zopiclone in the treatment of insomnia, increasing total sleep time as well as sleep efficiency according to polysomnography.

ABSTRACT

Os linfomas são uma neoplasia maligna, os quais se originam nos linfonodos, que estão envolvidos no combate às infecções.Essa neoplasia teve o maior crescimento na incidência entre as doenças oncohematológicas. O objetivo do estudofoi avaliar os possíveis sintomas gastrointestinais apresentados pelo paciente durante o tratamento e relacioná-los com aingestão alimentar e estado nutricional. Foi realizada a avaliação subjetiva global, índice de massa corporal, área musculardo braço e avaliada o consumo alimentar, assim como presença de sintomas gastrointestinais em portadores de linfoma,submetidos à quimioterapia ambulatorial. A amostra foi composta por 30 pacientes do Serviço de Oncologia Clínica doHospital Erasto Gaertner do Paraná, entre os meses de maio a junho de 2014, sendo 33,3% com linfoma de Hodgkin e66,7% com linfoma não Hodgkin. Segundo a ASG 100% dos pacientes foram considerados como nutridos, porém 46,7%dos pacientes apresentaram perda ponderal. Segundo a AMB, 57% dos pacientes apresentaram algum grau de desnutrição.Em relação ao IMC, 26,7% dos adultos estavam eutróficos, 30% com sobrepeso, 13,3% obesidade e nenhum com desnutrição,já nos idosos 6,7% estavam abaixo do peso, 6,7% eutróficos e 16,7% com excesso de peso. Houve diminuição naingestão alimentar por parte dos pacientes após início da quimioterapia, sendo náuseas, disgeusia, xerostomia e obstipaçãoos sintomas mais frequentes. Considerando os sintomas gastrointestinais que interferem diretamente no padrão alimentare a perda ponderal ocasionados pelo tratamento quimioterápico, os pacientes oncohematológicos devem ser consideradospacientes em risco nutricional e receber acompanhamento nutricional especializado, visando minimizar seus efeitoscolaterais adversos e melhorar sua qualidade de vida.


Lymphomas are a malignant neoplasm which arises in the lymph nodes, which are involved in fighting infection. This cancerhad the highest increase in incidence among oncohematological diseases. The objective of the study was evaluate the possiblegastrointestinal symptoms presented by the patient during treatment and relates them to food intake and nutritionalstatus. The subjective global assessment was performed; body mass index, arm muscle area and assessed food intake, aswell as presence of gastrointestinal symptoms in patients with lymphoma undergoing outpatient chemotherapy. The sampleconsisted of 30 patients of the Oncology Clinic Erasto Gaertner Hospital of Paraná service, between the months of Mayand June 2014, 33.3% with Hodgkin’s lymphoma and 66.7% with non-Hodgkin lymphoma. According to ASG 100% ofpatients were considered nourished, but 46.7% of patients had weight loss. According to the AMB, 57% of patients hadsome degree of malnutrition. In the BMI, 26.7% of adults were eutrophic, 30% were overweight, 13.3% were obese andnone with malnutrition, in relation to the elderly 6.7% were underweight, 6.7% were eutrophic and 16.7 % overweight.There was a decrease in food intake by some patients after chemotherapy initiation, being nausea, dysgeusia, dry mouthand constipation the most frequent symptoms. Considering the gastrointestinal symptoms that interfere directly in the dietarypattern and weight loss caused by chemotherapy, the onco hematological patients should be considered at nutritional riskand receiving specialized nutritional monitoring to minimize its adverse side effects and improve their life quality.

ABSTRACT

Diseases such as diabetes mellitus, hypertension, chronic glomerulonephritis, uropathy and autoimmune diseases are considered the most frequent causes of renal failure. The aim of this study was to investigate oral manifestations caused by chronic renal failure. Oral manifestations of chronic renal failure include dysgeusia, stomatitis, decreased salivary flow rate, xerostomia and parotitis; moreover, dental oriented conditions include narrowing of pulp chamber, enamel abnormalities, tooth loss and periodontal disease. The most frequent dilemma in patients with Kidney failure is Renal Osteodystrophy. Chronic renal failure could be treated by renal replacement therapy, hemodialysis and peritoneal dialysis. These are methods of removing nitrogenous and other toxic byproducts of metabolized blood. Oral maladies due chronic renal failure occur because of immunosuppressive medications. However, this kind of therapy could be challenging as it causes gingival inflammation due to increased plaque deposition; which in turn affects the periodontal tissues. A good oral hygiene may reduce the risk of oral infections in chronic renal failure patients. The awareness about kidney related diseases is often neglected; however, the number of patients with oral manifestations related to chronic renal failure is miraculous

BACKGROUND AND OBJECTIVES: After bilateral mastoidectomy, taste change is common because of injury to chorda tympani nerve. We aimed to understand about the appropriate interval of sequential bilateral mastoidectomy. SUBJECTS AND METHOD: Retrospective review was carried out for 25 patients who underwent sequential bilateral mastoidectomy from March 2004 to November 2011. RESULTS: Among the 25 patients, there were 19 patients whose chorda tympani nerve was injured after bilateral mastoidectomy, and eight of those 19 patients complained of taste disturbance. The rates of dysfunction according to operation interval were within 6 month (55%, 5/9), between 6 and 12 months (33%, 2/6), and above 12 months (10%, 1/10). Compared to canal wall up mastoidectomy, for canal wall down mastoidectomy, taste disturbance was more common (p=0.001). CONCLUSION: The result supports that at least 6 months interoperative period for bilateral chronic otitis media may be needed to avoid taste disturbance. A further large study may validate this result.

ABSTRACT

A radioterapia em região de cabeça e pescoço provoca inúmeras sequelas ao paciente irradiado, afetando o sistema estomatognático e com repercussões sistêmicas importantes. As sequelas da radiação ionizante podem ser extensas e, algumas vezes, permanentes, em especial nas glândulas salivares e no tecido ósseo. É relevante que o cirurgião dentista tenha conhecimento das reações adversas e das formas adequadas de prevenção e tratamento para amenizar o desconforto e melhorar a condição de vida do paciente irradiado. Portanto, a conscientização e motivação deste paciente, com a promoção de saúde oral através da adequação do meio bucal e orientações sobre ações preventivas, são essenciais para se obter o melhor prognóstico.


Head and neck radiotherapy causes countless sequelae in irradiated patients, affecting the stomatognathic system, with significant systemic implications. Sequelae of exposure to ionizing radiation may be extensive and sometimes permanent, particularly in the salivary glands and bone tissue. It is of utmost importance that the surgeon dentist be aware of adverse reactions and appropriate forms of treatment to alleviate discomfort and improve the quality of life of the irradiated patient. Therefore, awareness and motivation of the patient, with promotion of oral health through the adaptation of the oral environment and guidance on preventive measures are essential to get a better prognosis.

ABSTRACT

Objetivo: avaliar fatores associados ao diagnóstico de ardência bucal como xerostomia, hipossalivação, sintomas depressivos, ansiedade, transtornos de sono, doenças sistêmicas e uso contínuo de medicamentos. Métodos: foram avaliados 22 pacientes atendidos em nível ambulatorial através de escalas validadas e questionário sociodemográfico, seguido de medidas de fluxo salivar espontâneo e estimulado através do método de expectoração salivar. Na análise estatística descritiva foi utilizado SPSS 16.0. Resultados: a amostra foi composta por 4 homens e 18 mulheres com idade média de 61,6+2,83 anos. As médias de fluxo salivar espontâneo e estimulado foram 0,27+0,06 ml/min e 0,84+0,08 ml/min, respectivamente. A hipossalivação esteve presente em 54,5% dos casos, sendo que a xerostomia foi referida por 31,3% dos pacientes. Sintomas depressivos leves foram observados em 22,7% da amostra. Na avaliação de ansiedade-traço 31,8% apresentaram sintomas de ansiedade moderada, 63,6%, sintomas de ansiedade elevada e 4,5%, ansiedade muito elevada. Quanto à avaliação de ansiedade-estado 27,3% apresentaram sintomas de ansiedade moderada e 72,7% sintomas de ansiedade elevada. Setenta e sete por cento apresentaram alterações de sono. Os exames sorológicos não apresentaram alterações significativas. Conclusão: estes resultados sugerem associação entre os parâmetros comportamentais avaliados e os sintomas bucais, sendo necessário o aumento da amostra para que possamos confirmar estatisticamente essa tendência.


Aim: to evaluate factors associated with the diagnosis of burning mouth, such as dry mouth, hyposalivation, depressive symptoms, anxiety, sleep disorders, systemic diseases, and continuous use of medications. Methods: twenty-two patients recruited from outpatient clinics were assessed using validated scales and a sociodemographic questionnaire. Patients’ spontaneous and stimulated salivary flow rates were also measured using saliva expectoration. SPSS 16.0 was used in the descriptive statistical analysis. Results: the sample consisted of 4 men and 18 women whose mean age was 61.6±2.83 years. The mean of spontaneous and stimulated salivary flow rates were 0.27+0.06 mL/min and 0.84+0.08 mL/min, respectively. Hyposalivation was present in 54.5% of cases, and dry mouth was reported by 31.3% of patients. Mild depressive symptoms were observed in 22.7% of the sample. When assessing anxiety-trait, we found that 31.8% had moderate symptoms of anxiety, 63.6% had symptoms of high anxiety, and 4.5% had very high anxiety. The assessment of anxiety-status showed that 27.3% and 72.7% had moderate and elevated anxiety symptoms, respectively. Seventy-seven percent had sleep disorders. Serological tests showed no significant changes. Conclusion: Our findings suggest an association between the behavioral parameters evaluated and the oral symptoms, indicating the need for a larger sample to confirm this trend using statistical analysis.
